Who can register the death?

The death can be registered by any of the following people:

A relative of the deceased
Someone who was present at the death
An administrator from the hospital or other institution
The person making the funeral arrangements


Information you will need to provide the registrar about the deceased include

Full name, and maiden name where applicable
Date of birth
Last address
Occupation of the deceased and the deceased’s spouse or partner, if applicable
Confirmation of whether the deceased was receiving a state pension or any state benefits


What to take to the Register Office

Medical certificate of cause of death issued by the doctor. (If there has been a post mortem, the coroners office will advise what will happen).
Money for the purchase of the certified death certificates

Where possible, and if applicable, it is also helpful to take the deceased’s:

NHS medical card
Birth certificate
Marriage or civil partnership certificate
Disabled blue badge and concessionary travel pass
